BACKGROUND CHECK CONSENT AND CONFIDENTIALITY AGREEMENT
At WU&PWK we serve the homeless in our communities as well as minors between the ages of 6-17 years old. In order to effectively serve and protect the children that we’re leading, we may require you to undergo a routine background check for additional safety measures. If you agree to allow us to perform a background check, you simply check the box below.(If you do not consent to a background check or have a negative mark on your background check, it does not mean you will not be accepted into our volunteer program. You may be limited to activities and events that exclude children.)
